Support to help you live successfully & independently

In its simplest terms, Carlyle Place at Home provides support to help you live successfully and independently in your home for as long as possible. Upon entering into a membership agreement, a “team” is created including you, your family, your doctor, and a Professional Care Coordinator to determine what services you need. Our plans have a different levels, allowing you to choose what package of services best meets your needs. Here are just a few of the services offered:

  • Emergency response system (24-hour emergency pendant)
  • Home safety inspection
  • Light housework, mobility and assistance with daily living
  • Medication reminders to keep on a consistent and safe schedule
  • In-home meal preparation
  • Personal support and advocacy to assist with medical appointments and errands
  • Wellness and nutrition assessment and consultation

In Home Care

Upon starting your membership, you’ll meet with a Personal Care Coordinator to choose from the following list of in-home services.

  • Emergency response system (24-hour emergency pendant)

  • Light housework, mobility and assistance with daily living such as paperwork, scheduling, etc.
  • Medication reminders to keep on a consistent and safe schedule
  • In-home meal preparation
  • Personal support and advocacy to assist with medical appointments and errands
  • Wellness and nutrition assessment and consultation*

The following vendors have been pre-screened for quality and fairness by Carlyle Place at Home. Additionally, being a member of our program makes you eligible to receive special discounts from these reputable businesses.

Out of Home Care

Carlyle Place at Home brings the benefits of a continuing care retirement community right to your doorstep. However, you also have access to some of the best facilities the Carlyle Place senior community has to offer. Members are encouraged to visit Carlyle Place’s Renaissance Wellness Center for a full health and fitness consultation. Our certified fitness professionals are specifically trained to assess your individual needs and recommend a customized routine just for you. From there, you are free to take this wellness information back home with you and incorporate it into your daily life.

When you enter into a membership agreement with Carlyle Place at Home, you have the security of knowing your healthcare needs will be met even when your circumstances change. When or if the time comes that you can no longer stay in your home, you have the advantage of having developed personal relationships with Carlyle Place at Home Personal Care Coordinators. These coordinators not only know you, but they know the Carlyle Place network of providers and can find the appropriate fit for your need. The Carlyle Place Health Centers provide dignified, compassionate around-the-clock care.

  • Harrington House, Skilled Nursing Center

Other Benefits

This membership also means you are now a part of the larger Carlyle Place community. Although you have opted to have “at home” services, you are encouraged to visit Carlyle Place and participate in our select programs.

  • Take part in seminars and book discussions featuring current hot topics such as aging successfully, improving balance to avoid falls, etc.
  • Learn about new therapies and approaches to handling chronic disease and memory care.

You are welcome and even encouraged to visit the community and attend the programs designed to help you embrace your life to the fullest. The choice is yours to select what interests and appeals to you.
